Assessing Individual Spiritual Needs
This self-assessing form provides the individual a tool to assist in determining some of the areas of spiritual needs being met and those needing attention. It may also be used by counselors to assess spiritual needs for clients, individually or in groups, in clinical settings while adhering to standard rules of privacy and confidentiality.
Two separate results may be obtained from the completed form: (1) A list of specific spiritual needs being underserved, and (2) an overall evaluation of the composite spiritual needs rated as “adequate”, “moderately adequate”, “moderately inadequate” and “inadequate”.
